Public Records Request Policy
The Department of Forestry and Fire Management requires a Public Records Request form be filled out for all applicable requests.
Forms can be submitted via email, snail mail - 1110 W. Washington Ste. 100, Phoenix, 85007, - or in person.
To submit public records requests for the Office of the State Fire Marshal please email them directly.
For all media or other PRR requests, please submit them to the agency's Public Affairs Office.
