Public Records Request Policy
The Department of Forestry and Fire Management requires a Public Records Request form to be filled out for all applicable requests. All requests must be submitted to the Public AffaIrs division. Forms can be emailed, mailed - 1110 W. Washington Ste. 100, Phoenix, 85007 - or submitted in person.
*If the Public Records request pertains to the Office of the State Fire Marshal - you can submit that directly to Office of the State Fire Marshal Public Records Request.
